Medical Professional Show Application

This Show (“Agreement”) is made effective between DQ Surgery Bar LLC DBA as “Dollz & Queenz” Surgery Bar and said Professional:

By checking the box below you agree to the following:

You are bound by legal practices according to the license(s), certification(s), or degree(s), that You currently have, to maintain an ethical, and logical approach within your scope of practice when responding to guests of the show. You further agree to discuss the specific area, & only that of the specific area as it relates to your topic, and or the topic you chose to present on the show in relation to your specialty area. I understand that any false information provided by me in this agreement, will not be accepted and will terminate my approval to appear in any future “Dollz & Queenz” DQ Surgery Bar Shows.

DQ Surgery Bar LLC DBA as “Dollz & Queez” Surgery Bar reserves the right not to be held responsible for any photos provided by said professional to have viewed on the show. Note: All show photos to be displayed must be accompanied by a photo release and a copy provided to DQ Surgery Bar LLC DBA as “Dollz & Queenz” Surgery Bar prior to being presented on the show. *(Professional Guest Referral Application) is only indicated if you choose to provide your own guest. If you choose not to provide your own guest. DQ Surgery Bar LLC DBA “Dollz & Queenz” Surgery Bar will provide a guest for you based upon the show database of approved applicants requesting to appear on the show.

A one time application processing fee of $25 is required at the time of your Show Professional Agreement submission. (All payments are non-refundable). Payments are securely accepted by the use of Zelle (to the business phone number) or PayPal. Once payment is received and processed you will be notified via email within 14 days of your future date to appear on the show. DQ Surgery Bar DBA “Dollz & Queenz” Surgery Bar reserves the right to deny any application as required by law.

DQ Surgery Bar LLC DBA as “Dollz & Queenz” Surgery Bar gladly extends an invitation to you & your company/business/practice to return to the show in the future to discuss another topic of interest. Due to multiple options of cosmetic surgery/ bodily enhancement treatments currently available, and the need for each marketing topic. You will not be required to submit a new application due to applications remaining on file for approximately 1 year once submitted. Please note: Due to the agreement in reference to applications being kept on file for 1 year. DQ Surgery Bar LLC DBA “Dollz & Queenz Surgery Bar hereby requests that any new information as it relates to address, or phone numbers etc be emailed to: dqsurgerybar@gmail.com in order to update your application.
