Photography, photo editing, and photo production: digital photography, digital photo editing, and digital photo production: and web design will be used for marketing and show production purposes.
The picture image(s) provided is that of the applicant listed on this agreement and consents to being a subject of marketing; and show productions in media. The picture image(s) of applicant will allow “DQ SURGERY BAR DBA “DOLLZ & QUEENZ” SURGERY BAR and the production Studio(s) that are hired by such to use photo(s).
The applicant concurs that all photo(s) images are that of themselves.

WHEREBY:

DQ Surgery BAR LLC DBA “DOLLZ & QUEENZ” SURGERY BAR IS RELEASED OF LIABILITY: For good and valuable consideration herein acknowledged as received, the applicant releases the photo image(s) and assigns permission to license all images in any media for any purpose which may include, among others, advertising, promotion, marketing, and packaging for any product or service. The applicant agrees that any images may be combined with other images, text and graphics, cropped, altered, and modified.

DURATION OF AGREEMENT: The applicant acknowledges and agrees that this Agreement is binding on all heirs and assigns. The applicant acknowledges and agrees that this Agreement is irrevocable, worldwide and perpetual.

This agreement contains the entire agreement between the parties to this release and the terms of this Agreement are contractual and not a mere recital.
